Wegovy and Mounjaro are now part of everyday UK health conversations. They can support significant weight loss, but many people have the same sensible question:

Could some of that weight come from muscle?

The short answer is yes. That does not mean these medicines “burn muscle” in a unique or inevitable way. Whenever you lose a meaningful amount of weight, your body can lose a mixture of fat, water and lean tissue. Your job is to give your muscles a reason to stay , and enough nutrition to support them.

TL;DR:

  • GLP-1 medicines can lead to some loss of lean mass during weight loss.
  • “Lean mass” is not the same as skeletal muscle. It can include water, organs, bone and other non-fat tissue.
  • Eat regular protein-rich meals, unless a qualified healthcare professional has advised you otherwise.
  • Add resistance training at least twice weekly, starting at a level you can manage.
  • Track strength, energy and everyday function , not only the number on the scales.
  • Blood biomarkers can add context, but they cannot directly measure how much muscle you have.
  • Speak with your prescriber, GP or a qualified dietitian before changing your medication, diet or exercise plan.

Can GLP-1 medicines cause muscle loss? Yes. Weight loss with Wegovy or Mounjaro can include some lean mass, but the amount varies. Protein, resistance training and professional support may help protect muscle during treatment.

Why can weight loss include lean mass?

Your body is not a simple fat-burning machine. It is more like a house being renovated while you are still living in it. When food intake falls and your body weight drops, your body uses stored energy. Some comes from fat. Some can come from lean tissue.

Body-composition studies of semaglutide and other GLP-1-based medicines show that fat loss usually makes up most of the weight lost, while lean mass can also fall. However, “lean mass” is a broad measurement. It can include muscle, water, organs, bone and other tissue. It is not a direct reading of your biceps, legs or grip strength.

A review of semaglutide and lean mass research found that results varied between studies. The measurement method, starting health, amount of weight lost and level of activity all made a difference.

A separate review explains why the phrase “muscle loss” needs care. Changes in lean mass do not always mean the same thing as a loss of useful muscle function. Read the review of GLP-1 medicines and muscle health for more detail.

The practical point is simple: protecting strength and physical function matters more than chasing a single body-composition number.

STEP 1: Make protein easier to eat

Wegovy and Mounjaro can reduce appetite. That is part of how they work, but it can also make it easy to eat too little overall , including too little protein.

Do not wait until the end of the day to discover that you have barely eaten. Build protein into your first meal and repeat it across the day.

Useful options include:

  • Eggs
  • Greek yoghurt or cottage cheese
  • Fish or lean meat
  • Tofu, tempeh or beans
  • Lentils and chickpeas
  • Milk or fortified plant-based alternatives
  • Protein supplements, if a qualified professional agrees they are suitable for you

Research in people undergoing weight loss often discusses protein intakes above the basic adult reference level. A commonly used evidence-informed range is around 1.2–1.6 g per kilogram of body weight per day, but this is not a universal prescription. The right amount depends on your size, age, activity, medical history and current diet.

If you have kidney disease, liver disease, diabetes, a history of eating difficulties or another long-term condition, ask your GP or dietitian to help you set a safe approach. Do not copy a high-protein plan from social media.

Try to spread protein across meals rather than eating almost all of it at dinner. Think of it like watering a plant. One huge bucket once a week is less practical than regular care.

If nausea, vomiting or early fullness makes food difficult, tell your prescriber. Do not simply push through ongoing symptoms or change your dose yourself.

STEP 2: Give your muscles a clear reason to stay

Walking is useful. So is cycling. General movement supports your health and helps you stay active during weight loss.

However, resistance exercise gives your muscles a more direct message: you are still needed.

The NHS England guidance on weight-management injections says Wegovy and Mounjaro should be used alongside a reduced-calorie diet and regular physical activity. It does not give a GLP-1-specific sets-and-repetitions plan, so keep your starting point realistic.

Aim to include muscle-strengthening activity at least twice a week if you can do so safely. You might use:

  • Body-weight squats or sit-to-stands
  • Wall press-ups
  • Resistance bands
  • Light dumbbells
  • Gym machines
  • Supported lunges
  • Carrying and lifting exercises

Start with a manageable level. Increase repetitions, resistance or control gradually. You do not need to train like a professional athlete. A consistent, modest session is more useful than one heroic workout followed by ten days of soreness.

If you are new to training, have joint pain, feel unsteady or have been inactive for a long time, ask a qualified healthcare professional or exercise professional for guidance. Supervision is not a sign that you are failing. It is simply a sensible way to learn the movements.

STEP 3: Monitor function, not just body weight

The scales can tell you that your total weight is changing. They cannot tell you exactly what has changed.

Keep an eye on practical signs such as:

  • Is your strength staying broadly steady?
  • Can you climb stairs as usual?
  • Can you stand from a chair without using your hands?
  • Are normal shopping bags suddenly much harder to carry?
  • Are you recovering from training, walks or daily tasks?
  • Are you feeling persistently weak, dizzy or unusually exhausted?

These are not diagnostic tests. They are useful observations to share with your prescriber or GP.

A home body-composition scale may show changes in “muscle mass”, but hydration can move the number up or down. Treat it as a trend, not a verdict. It is more like a weather forecast than a photograph.

If your strength or daily function is falling quickly, arrange a conversation with a qualified healthcare professional. Your calorie intake, protein, exercise, dose and wider health all need to be considered together.

STEP 4: Understand what blood biomarkers can , and cannot , tell you

Blood testing can help you build a wider picture while you are working with your prescriber. It cannot directly measure your muscle mass.

Some markers can be influenced by factors linked to nutrition, hydration, training or body composition:

  • Creatinine: affected by muscle mass, hydration and protein intake, as well as kidney filtration.
  • Urea: influenced by protein breakdown, hydration and kidney function.
  • ALT and AST: can change after hard exercise, as well as with liver-related causes.
  • CRP: may rise with infection, injury or recent intense training.
  • Ferritin: reflects stored iron but can also be affected by inflammation.
  • HbA1c: shows your average blood sugar over recent months. It does not measure muscle.
  • Vitamin D, B12 and thyroid markers: may provide useful context when discussed alongside symptoms and medical history.

This is why a blood result should not be read in isolation. One number is a dashboard light, not the whole engine.

Vitall Check’s Performance Monitor blood test includes a broad selection of metabolic, organ, nutritional, blood and hormone markers. Samples are processed by UKAS-accredited laboratories using NHS-grade standards, with results presented in plain English.

If you use an at-home test, prepare properly. Stay hydrated, follow the official “how to collect your sample” guide and avoid heavy exercise beforehand if the instructions advise this. Finger-prick collection is convenient, but some people find it difficult. If you cannot collect enough blood or feel faint, stop and seek support rather than repeatedly trying while distressed.

When should you speak with a healthcare professional?

Contact your prescriber, GP or another qualified healthcare professional if:

  • You are struggling to eat or drink because of nausea, vomiting or diarrhoea.
  • You feel faint, persistently weak or unable to manage normal activities.
  • Your strength is dropping quickly.
  • You want to change your dose or switch between Wegovy and Mounjaro.
  • You have kidney disease, liver disease or another condition affecting nutrition or exercise.
  • You are pregnant, planning pregnancy or breastfeeding.
  • You have severe or persistent abdominal pain, especially pain that spreads to your back.

The MHRA’s guidance on GLP-1 medicines explains that these medicines are prescription-only and should be obtained through a legitimate pharmacy following a healthcare professional’s assessment.

The takeaway

GLP-1 muscle loss is a real concern, but it is not a reason to panic. Some lean mass can be lost during any substantial weight-loss phase. Your best practical tools are regular protein-rich meals, sensible resistance training, adequate hydration and attention to strength and everyday function.

Blood biomarkers can help you arrive at your next GP appointment with better questions and useful context. They cannot diagnose muscle loss or replace clinical care.
