Hormones rarely work alone. They act more like a group chat: testosterone, oestradiol, progesterone, thyroid hormones, cortisol, SHBG and DHEA are all sending messages at the same time.

That is why a single result can be hard to understand without timing, symptoms and context.

An at-home hormone blood panel can give you useful information to discuss with your GP. It can help you arrive informed, with clear results rather than a vague list of concerns. But it is not a diagnosis, and one blood test cannot explain every change in energy, mood, sleep, libido, weight or menstrual cycle.

What does an at-home hormone blood panel test? An at-home hormone blood panel measures selected hormones and related markers, such as testosterone, oestradiol, progesterone, cortisol, thyroid hormones, SHBG and DHEA-S, using a small blood sample collected at home and analysed by a laboratory.

What is an at-home hormone blood panel?

An at-home hormone blood panel is a set of blood tests collected outside a clinic, usually with a finger-prick sample. The sample is sent to a laboratory, where the selected markers are measured.

Which hormones can be included?

The right panel depends on your question. A large panel is not automatically more useful. It is better to choose markers that relate to your symptoms, medication, age, sex, cycle stage and health history.

Testosterone

Testosterone is present in everyone, although average levels differ between individuals and change with age.

In men, testosterone is often considered alongside symptoms such as reduced sexual desire, fewer morning erections, erectile difficulties, reduced strength, low mood or persistent tiredness. In women, testosterone can also be relevant to sexual function, energy and androgen-related symptoms.

Testosterone follows a daily pattern. UK NHS pathology guidance recommends measuring it before 11am, using a fasting sample, because food intake and the time of day can affect the result. A low result should not be treated as a final answer. The North Bristol NHS protocol states that a result below the laboratory reference range should be confirmed on two occasions, with SHBG helping to clarify borderline results. SHBG

Sex hormone-binding globulin, or SHBG, is a protein that carries sex hormones through your blood.

Picture SHBG as a fleet of taxis. Some hormones are travelling in the bloodstream, but not all are freely available to tissues. SHBG helps explain why two people with a similar total testosterone result may have different calculated free testosterone results.

SHBG can be affected by factors including thyroid function, liver health, body composition, medicines and oestrogen exposure. It is often most useful when read alongside testosterone rather than on its own.

Oestradiol

Oestradiol is the main form of oestrogen measured in many blood tests. It is important for reproductive health and also plays a role in bone, mood and sexual health.

Oestradiol changes during the menstrual cycle. It can also change during perimenopause, pregnancy, hormonal contraception and hormone replacement therapy.

For people aged 45 or over with typical menopause symptoms, NICE advises that perimenopause and menopause are generally identified from symptoms and menstrual history without confirmatory laboratory tests. That does not mean oestradiol is never tested. It may be relevant in younger people, unusual or unclear symptoms, fertility investigations, or when a clinician is monitoring a specific treatment.

Progesterone

Progesterone rises after ovulation. A progesterone test can therefore be used in some fertility investigations to check whether ovulation has occurred.

Timing matters more than the label “day 21”. For a 28-day cycle, day 21 is about seven days before the next period. If your cycle is longer or shorter, the testing day may be different.

Gloucestershire Hospitals NHS guidance recommends collecting the sample seven days before the next predicted period. It also explains that a progesterone result of around 30 nmol/L or more at that point can be a guide that ovulation has occurred, although it is not an absolute measure. A single progesterone result can be difficult to interpret if your cycle is irregular, your predicted period date is uncertain, or you use hormonal contraception.

Cortisol

Cortisol is made by your adrenal glands. It helps your body respond to changes in demand, including illness, physical activity and emotional pressure.

Cortisol follows a daily rhythm, rather like a clock with the volume turned up in the morning and down later in the day. Gloucestershire Hospitals NHS guidance says that blood samples for an initial screen are best collected at 9am.

A home cortisol result is not a complete investigation for every adrenal condition. Dynamic tests, such as a short Synacthen test, may be needed in clinical care. Thyroid hormones

Thyroid testing commonly starts with:

  • TSH: the signal from your brain telling the thyroid how hard to work.
  • Free T4: one of the main thyroid hormones in your bloodstream.
  • Free T3: sometimes added when clinically relevant.
  • Thyroid antibodies: sometimes included when autoimmune thyroid disease is being considered.

The thyroid is a little like your body’s heating and power-control system. If the control signal is too high or too low, your energy, temperature, heart rate, digestion, mood and menstrual cycle may be affected.

NHS information explains that tests for an underactive thyroid usually measure TSH and T4. DHEA and DHEA-S

DHEA is produced mainly by the adrenal glands. Blood testing often measures DHEA-S, the more stable sulphate form.

DHEA-S can be included in broader hormone panels, particularly when looking at androgen-related patterns or adrenal hormone production. It should not be used as a stand-alone explanation for tiredness, stress or changes in body composition.

A result outside the laboratory reference range needs context. Your age, sex, symptoms, medicines and other hormone results all matter.

Hormone testing for men: which markers may be useful?

A male hormone panel may include:

  • Total testosterone
  • SHBG
  • Calculated free testosterone
  • LH and FSH
  • Oestradiol
  • DHEA-S
  • Thyroid markers
  • Cortisol
  • Prolactin, depending on the panel and clinical question

Testosterone, SHBG, LH and FSH work together like a thermostat, control signal and output reading. Testosterone shows part of the output. LH and FSH show signals from the pituitary gland. SHBG helps explain how much testosterone may be available for use.

If you lift weights, train hard or follow a demanding programme, remember that context matters. Heavy training, poor sleep, recent illness, low energy intake and dehydration can affect how you feel and may influence some blood markers. A higher creatinine result in a muscular person, for example, may need to be read differently from the same result in someone with a different body composition. Hormone results should be discussed in the context of your training load rather than treated as a scoreboard.

For men concerned about testosterone, collect the sample early in the morning and follow any fasting instructions. If a result is low or unexpected, arrange a GP discussion and expect that repeat testing may be needed.

Hormone testing for women: which markers may be useful?

A female hormone panel may include:

Menstrual-cycle timing

For cycle-related testing, write down:

  • The first day of your last period
  • Your usual cycle length
  • Whether your cycle is regular or irregular
  • Any hormonal contraception or HRT you use
  • The date your next period is expected

This information is not admin clutter. It is part of the result. A hormone level without cycle timing is like receiving a weather reading without knowing whether it was taken in January or July.

Progesterone is usually timed about seven days before your next expected period. Oestradiol, FSH and LH may be tested at different stages depending on the reason for testing, so follow the instructions for your specific panel.

Perimenopause and menopause

If you are aged 45 or over and have typical symptoms such as hot flushes, night sweats, cycle changes, vaginal dryness, sleep problems or mood changes, NICE says diagnosis is generally based on symptoms and menstrual history rather than a hormone blood test.

Blood tests may still be relevant if symptoms are unusual, you are under 45, there is a concern about another condition, or a clinician is investigating premature ovarian insufficiency. NICE advises that premature ovarian insufficiency under 40 should not be diagnosed from one blood test alone; it requires symptoms and raised FSH on two blood samples taken four to six weeks apart.

How to prepare for an at-home hormone blood test

Preparation helps reduce avoidable problems. It cannot remove normal biological variation, but it can make the sample easier to collect and the result easier to understand.

STEP 1: Check the timing

Read your panel instructions before collecting your sample.

  • Testosterone: usually early morning.
  • Cortisol: commonly around 9am.
  • Progesterone: about seven days before your next expected period.
  • Thyroid markers: usually less dependent on time of day, but follow your kit instructions.
  • Repeat tests: collect them at a similar time if you are comparing results.

Do not stop prescribed medication unless your GP or another qualified healthcare professional tells you to.

STEP 2: Prepare your body and space

  • Drink water before testing.
  • Avoid alcohol for 24 hours.
  • Avoid intense exercise before collection.
  • Follow any fasting instructions. Water is usually allowed, but check your panel.
  • Ask about biotin supplements, as high doses can interfere with some hormone and thyroid assays.
  • Choose a clean, well-lit surface.
  • Have a plaster nearby.

STEP 3: Warm your hands

Cold fingers can make collection frustrating. Wash your hands in warm water and dry them properly. If needed, hold your hand under warm running water for around two minutes.

STEP 4: Follow the official collection method

Use the supplied safety lancet on the side of your middle or ring finger. Wipe away the first drop if your instructions tell you to. Keep your hand below your heart and gently massage from the base of the finger towards the tip.

Do not squeeze aggressively. It can mix tissue fluid with the sample and may damage blood cells.

STEP 5: Seal and post promptly

Fill the tube to the marked line. Seal it correctly, place it in the biohazard bag and use the prepaid return packaging. The sampling guide recommends posting the sample on the same day.

How to read your hormone results

Start with the basics:

  1. Check the result and unit.
  2. Check the laboratory reference range.
  3. Check the sample date and time.
  4. Check your menstrual-cycle day, if relevant.
  5. Note medicines, contraception, HRT, supplements and recent illness.
  6. Read related markers together.

Do not compare your number with a screenshot from social media or a different laboratory. Reference ranges can differ because laboratories use different equipment, methods and populations.

Your Insight Report is a manual for understanding the report in plain English. It can explain what a marker does, where your result sits within the laboratory’s stated range and what information may be useful for your next GP appointment.

It does not diagnose a condition. It does not tell you to start supplements, alter medication or change hormone treatment.

When should you speak to your GP?

Arrange a GP discussion if:

  • A result is outside the laboratory reference range.

Safety notes and limitations
